To complete the survey and enter the draw for prizes, you need to be a fee-paying subscriber to Pathway's Dial-up or LightPath/FastPath service. |

To be eligible for the draw, you will need to complete the survey, in full, by midnight on September 16, 2011. |

The online form permits only one entry per Username. |
| 4. |
Any prize won by a minor will be awarded in the name of the User's parent or legal guardian who is 21 years of age or older. |

The terms of the draw for prizes are available at https://www.pathcom.com/survey/survey_terms.php. Those entering the draw agree to abide by those terms. |

Those who do not have a fee-paying account are welcome to complete the survey but are excluded from participating in the draw for prizes. |

The draw will take place on September 19, 2011, and the subsequent winners will be contacted via phone and/or their Pathway email address. Winners must claim their prize in person at the Pathway office. |
